This is going to be a two part lesson. In this first part we start off talking about what vasopressors are and why they are used for our patients. We then get in to the actions of vasopressors on various receptor sites and explain the differences between these receptors. From there we move on and cover the 6 main vasopressors and talk about the specific receptors in which they act as well as some useful information relating to dosing and onset of action. We finish out the lesson with a chart comparing these vasopressors in one spot so you can have a better understanding of these differences between each pressor. The next part of this series we will be covering very important tidbits of information that come in to play when managing patients on these medication. Our hope is that by the end of this lesson you will have a better understanding of what pressors are as well as the differences between the main various vasopressors that we use. Almost 6 million patients land in an intensive care unit every year, and for many, it marks a turning point in their lives. A substantial number of patients leave the ICU with newly acquired problems, from dementia to nerve disease. Medical leaders have developed new standards to reduce the use of drugs and get patients moving, but adoption has been slow. Special correspondent Jackie Judd reports.

We start off talking about what inotropes are, primarily a class of medications used to increase the contractility of our patients hearts. We also talk about the 2 primary ways in which medications can achieve this effect. Starting with a group called catecholamines, we cover how these medications cause an increase in our patients contractility, and thus cardiac output and then move in to talk about different medications within this class, covering the concentrations and dosages that you would expect to find. Of these medications, Dobutamine is the only one that is going to be used solely and primarily for this effect. Next we talk about a unique group within this class called the Phosphodiesterase Inhibitors and we explain how quite differently these medications can lead to an increase in our patients contractility, as well as the vasodilatory effect they have on blood vessels. Here we primarily talk about 1 medication called Milrinone, which is another drug that we use specifically for its inotropic effect. Opinion Video producer Alexander Stockton spent several days reporting at the Valleywise Medical Center in Phoenix. Two I.C.U. nurses wore cameras to show what it’s like to care for the sickest Covid patients a year into the pandemic. So many Americans have died in hospitals without family by their side, but they were not alone. Nurses brush patients’ teeth, change their catheters and hold their hands in their final moments. In just a year, we’ve lost half a million Americans to Covid-19. Vaccinations may be offering some relief, but inside I.C.U.s, nurses continue to contend with the trauma and grief of America’s carousel of death. These basic ventilator modes will form the core understanding of the ways in which we are supporting our patients breathing. We start off talking about the differences between the 2 mains types of ventilation, volume control and pressure control. In this lesson we are going to focus solely on volume control so we move on to talk about the different settings that we will find for our vents in this mode, as well as just a couple of the key bits of monitoring data that will be displayed on the vent. From there we move in to talk about the different basic types of volume control modes.
